which is more effective?? downward angle seems to be the easiest to hide, and most comfortable...is it as effective?? the instruction book said to stretch upwards...

I have checked all the documentation with the SizeGenetics, and it doesn't say that it will be any less effective if worn downwards.
It sounds to me as though they advise to wear the device upwards for comfort, and for discretion. It would seem easier to hide the device under clothiing if it is folded upwards toward the body, but I would say that there is no reason to avoid wearing downwards.
The best advice I can offer is to try it both ways, and see what works best for you.

Stretching against the pull of the ligament (downwards) will yield the best results.

I read 'upwards' too in the docs. Since this is a long haul exercise I also go for comfort - so down it is when ever possible! If that means a month more, at least it is a more comfortable month. I do vary it from up middle down when I can, but wear it mainly down.
