How to properly attach the SizeGenetic

I did not notice any instructions that came along with the device explaining exactly how to mount the device. My penis keeps slipping through the strap and I am not sure what I am doing wrong...

Try reading through all of the SG extender stickies.This comes up a lot on here.I only do manual exercises so i can't help much.
